As published by the 2014 AIChE Annual Safety in Ammonia Plants and Related Facilities Symposium

A brittle fracture failure of several HK-40 tubes occured in the convection section of an ammonia plant feed gas heater. A failure analysis and API 579 Fitness-for-Service assessment were performed to determine the mode of failure and to determine the remaining life of the HK-40 tubes. This information provided the operator with a sound basis to evaluate the business and safety risks of continued operation until long-term repairs and system improvements could be implemented.

Historically, inspections for IFR Type B failures required a manned entry into the vessel. A confined space entry of this nature requires extensive planning and must be supported with product transfer, supplied air and an extensive lockout/tagout (LOTO) effort. However, a new solution utilizing remote digital video inspection (RDVI) techniques is now available for single seal tanks to alleviate the challenges associated with IFR Type B inspections.

Previously reformer outlet pigtails were considered standalone components with a nominal 100,000 hour lifespan. With improvements in reformer tube metallurgy and manufacture, pigtails are now seen as a weak link and often require replacement before the reformer tubes and manifolds. There is a strong incentive to improve pigtail reliability and lifespan, taking into account all the key factors, from design to material selection, metallurgy, grain size distribution, quality control or inspection techniques, and all the major focal points during the life cycle of these components.
